Care Team
Our ears and hearts are open so that we can provide end-of-life care with trust, empathy, dignity and for the whole person and whole family. We are here for you and your family to listen and hear. We want to ensure you find the comfort and support you need during this time.
The care team at Hospice Wellington includes Team Lead Nurses, Registered Nurses, Registered Practical Nurses, Personal Support Workers (PSWs), Palliative Care Physicians, Psychosocial Spiritual Coordinator, Art Therapist, Volunteers, Pharmacist services and Complimentary Therapists. The Director of Care at Hospice Wellington is an RN with specialist education and experience in Palliative Care. There are two Registered staff and a two PSW's on each day shift which is from 8 a.m. to 8 p.m. daily and at night there are two Registered staff and PSW overnight from 8 p.m. to 8 a.m. to provide care and symptom management. Our Team Lead Nurses is here throughout the week to help manage referrals and activities on the residence floor. Hospice Wellington has access to, and support of a Palliative Care Physician 24/7 as required, and all staff have specialist Palliative care education and skills to deliver exemplary care. Hospice Wellington has a Clinical Resource Nurse who provides clinical care support and education to the care team and provides liaison with our Community and Hospital care providers.
The team works collaboratively to provide a truly holistic and person-centered approach to care by combining skills and knowledge within their roles and responsibilities. Hospice Wellington volunteers in the Residence are integral to the support we provide. Each member of the team strives to make a difference to those in our care and their families.
